Before Joanna, Ryan was married to Leigh Taylor Young on 28 February 1967. The former couple has a son named Patrick O'Neal (b. 14 September 1967). They divorced on 13 March 1974.

Ryan and his ex-girlfriend Farrah first met through Farrah's husband, Lee Major. Ryan requests Lee to look after his teenage daughter Tatum and in exchange, he requests Ryan to look after his wife, Farrah, while Ryan was filming in Los Angeles in 1979.

Ryan fell in love with Farrah and started to date in 1980. Soon after she finalized her divorce with Lee in 1982, Ryan and Farrah welcomed their first child Redmond O'Neal in 1985. Although the couple was never married, they were often caught in paparazzi and controversial.
